ELIGIBILITY:           Diagnosis of vision impairment by the person’s primary eye-care professional within the past three months.

 

DESCRIPTION:       Comprehensive low vision evaluation for persons with vision impairment or blindness, traumatic brain injury, stroke, and special populations with low vision; device recommendation, dispensing, and training. Networking and referral to other agencies for additional vision rehabilitation services for the patient when necessary. Financial assistance may be available.
